A Type 1 opt-out can only be recorded by the GP practice by you completing the form here (EXTERNAL DOWNLOAD LINK). You can email this to nencicb-tv.a83031-eds@nhs.net
A National Data Opt Out is set by the patient themselves using the online service, the telephone service, the NHS App, or 'print-and-post', completing a form by hand and sending it in.
